April 22 @ 3 pm: The RI MOLST Form: A Deep Dive
The Medical Orders for Life Sustaining Treatment form can be completed by a seriously ill patient & is honored across all healthcare settings. We will go through the form step by step & discuss the implications & when it may be appropriate for yourself or a loved one.
This program will also be available via Zoom. Look for the link on the Featured programs page of the library website
This program is led by Bonnie Evans. Bonnie is a geriatric nurse practitioner with experience in hospice and palliative care, an end-of-life doula, & grief counselor.
